What To Bring To My First Appointment?

To process your application and get you approved for Ryan White services and HIV medications as well as Insurance Assistance through the AIDS Drug Assistance Program (ADAP), please review the Eligibility & Enrollment Document Checklist found on our website at https://endhivnv.org/directory/eligibility-and-enrollment-checklist/
At your first appointment you’ll need to bring documentation to prove:
- HIV Diagnosis
- Proof of Identification
- Current labs
- Proof of Nevada Residency
- Proof of income level
- Proof of existing insurance coverage
To receive any of the services available through the Nevada Ryan White Programs, you must complete an Initial Eligibility Assessment*. This first assessment will collect all information and supporting documentation needed to determine your eligibility for the program.
Eligibility must be re-assessed six months after the first full assessment – the easiest way to do this is through the Six Month Self-Attestation Form (CGD 15-46) found on our website at https://endhivnv.org/directory/eligibility-and-enrollmenteligibility-and-enrollment-checklist/.
