What To Bring To My First Appointment?

To receive any of the services available through the Nevada Ryan White Programs, such as core and support services, medication assistance, and health insurance assistance, you must complete an initial Universal Eligibility Application Packet for Brand New Clients (18-04a). This first assessment will collect all information and supporting documentation needed to determine your eligibility for the program. At your first appointment you’ll need to bring documentation to prove:

  • HIV diagnosis
  • Nevada residency
  • Income level

Eligibility must be re-assessed approximately every six months after the first full assessment – during your birth month and half-birth month.
